For Hudson & Passaic County
CarePoint Health can now offer you and your family the option of quality healthcare where and when you need it.
What is covered?
Only specialty care is covered for veterans and TRICARE beneficiaries who choose CarePoint Health under the Patient-Centered Community Care program.
Under the Veterans Choice Program, CarePoint Health can provide a myriad of healthcare services including primary care, inpatient and outpatient specialty care, mental health care, limited emergency care and limited neonatal care for enrolled female service members. All services offered by community service providers must be pre-approved prior to treatment.
Who is eligible?
All Veterans and TRICARE beneficiaries who are currently enrolled in the VA healthcare system are eligible to participate in the Patient-Centered Community Care program.
To participate in the Veterans Choice Program, eligibility is limited to veterans enrolled in the VA healthcare system on or before August 1, 2014, or within five years of post-combat separation. Additional criteria include a wait time of more than 30 days from a preferred date or the date medically determined by a physician, residence that is more than 40 miles from the closest VA health care facility or extreme difficulty accessing a VA facility due to geographical challenges or that requires extensive travel.
How to get started!
Veterans interested in choosing CarePoint Health for specialty care should contact their nearest VA office. Veterans interested in scheduling an appointment and checking eligibility for the Veterans Choice Program should call 866-606-8198 or visit www.va.gov/opa/choiceact
TRICARE beneficiaries should contact their Primary Care physicians to discuss utilizing CarePoint Health for their specialty care needs.
CarePoint Health is proud to be an authorized provider of Patient-Centered Community Care and Veterans Choice Program Services in Hudson and Passaic County, making accessing healthcare easier and more convenient for veterans and TRICARE beneficiaries. With CarePoint Health, you now have access to superior medical care at Bayonne Medical Center, Christ Hospital in Jersey City, and Hoboken University Medical, as well as an extensive network of medical professionals in Passaic County.
Patient-Centered Community Care
Eligible veterans and TRICARE beneficiaries can now receive “out of network” medical care through the Patient-Centered Community Care (PCCC) program. PCCC provides access to specialty care through a comprehensive network of community-based, non-VA medical professionals when the VA cannot provide adequate care or must supplement care outside of its own facilities. With CarePoint Health as part of PCCC’s network, you can be certain that you and your family will receive quality medical care that is timely, convenient and close to home.
Veterans Choice Program
Eligible veterans who have inadequate access to VA medical care because of long wait times or distance constraints now have the option of seeking non-VA care through the Veterans Choice Program (VCP). As a participating VCP provider, CarePoint Health’s three facilities and its extensive network of medical specialists, along with a myriad of community-based health care practices in Passaic County, are now at your disposal.
