Our Approach

Having the right support team, surroundings and services to make the transition from hospital to home can speed up healing and improve outcomes. The Transitional Care Units at Hoboken University Medical Center and Bayonne Medical Center have 32 beds combined, offering private and semi-private rooms operating 24 hours a day, seven days a week.

You’ll find a team of caregivers dedicated to providing excellent, compassionate and diligent care at both Transitional Care Unit locations. While staying with us your individualized plan of care can include:

Admission to the Transitional Care Unit
Prospective candidates for the Transitional Care Unit must meet the following admission criteria:

  • Have had a minimum of a three-day stay in an acute care hospital (applies only to Medicare patients at Bayonne Medical Center)
  • Require skilled nursing or rehabilitation services five days per week
  • Expect to be ready for discharge in eight days
  • An acceptable admitting diagnosis documented in the medical record
  • Not currently a resident of a long-term care facility

In order for a patient to gain admission to our Transitional Care Unit, the attending physician must write an order for referral.

As a Medicare-approved service, we uphold the highest standards of care under the guidelines of Hoboken University Medical Center, Bayonne Medical Center and of licensed long-term care facilities.
